AI Search Visibility for Chiropractors

Patients are starting to ask ChatGPT, Google's AI Overviews, and Perplexity where to find a chiropractor before they ever type a search into Google the old way. Most practices have no idea whether they show up in those answers at all. Here's what actually determines it, and what it looks like when it works.

Short answer: AI systems recommend a practice when they can confirm, from multiple independent sources, that it exists, does the specific thing the patient asked about, and is trusted by real patients. That confirmation comes from five places: your Google Business Profile, your website's service pages, your structured data, your citation consistency across directories, and mentions of your practice by name outside your own website. Miss two or three of those and the AI has nothing solid to point to, so it recommends whichever competitor gave it more to work with.

What actually moves the needle

Five signals, in the order I check them

  1. 1. A complete Google Business Profile, with reviews that say something specific

    Every category, service, and attribute filled in, not just the required fields. And reviews that name a real condition or outcome beat five-star ratings with no text every time. “Great chiropractor” tells an AI system nothing. “Fixed my lower back pain after two visits, didn’t push a long treatment plan on me” is something it can actually match to a patient's question.

  2. 2. One dedicated page per real service

    A single page that lists ten services in one paragraph gives an AI system nothing to point to for any one of them. A practice with a real, specific page for spinal decompression, a real page for prenatal chiropractic care, and a real page for sports injuries gives the AI three separate things it can confidently recommend the practice for.

  3. 3. Structured data that describes the practice correctly

    Schema markup doesn’t make content rank higher by itself, but it removes ambiguity about what a page is and what it's about, which makes AI systems more confident citing it. Practices with clean schema get cited noticeably more often than ones without, not because the schema is magic, but because it removes guesswork.

  4. 4. The exact same name, address, and phone number everywhere

    AI systems don't reconcile “Smith Chiropractic” and “Smith Chiropractic Clinic, PLLC” as the same business the way a human would. If your listing is inconsistent across your website, Yelp, Facebook, and other directories, the AI can lose confidence you're a real, verifiable business at all.

  5. 5. Being named by someone other than yourself

    A real local publication, a real community mention, a real “best of” feature written by an actual outlet. This is the signal most practices have zero of, and it's the one that pushes an AI system from “this business exists” to “this business is worth recommending.”

Questions I actually get asked

Do I need to do anything different for ChatGPT versus Google?

Some of it overlaps and some doesn't. Google's AI Overviews are built on top of your regular Google ranking and your Google Business Profile, so the map-pack and SEO fundamentals still matter most there. ChatGPT and Perplexity pull more heavily from a different mix of sources: your reviews on other platforms, consistency of your business name and address everywhere it appears online, and whether real publications or directories mention you by name. A practice can rank well in Google Maps and still be invisible to ChatGPT, or the other way around, because the two systems aren't reading the same signals.

Why is my Google Business Profile suspended, and does that affect AI visibility?

Suspensions usually come from an edit that looked automated or suspicious to Google: a bulk update, a name change, or a login from an unfamiliar device or location. While it's suspended, your practice effectively disappears from Google Maps, and since Google's AI Overviews lean on your Business Profile more than almost any other source, you disappear from that surface too. Reinstatement is usually a straightforward appeal, but it can take days, which is why we treat GBP account health as something to monitor continuously, not just set up once.

Why is my website ranking so low even though I've had SEO work done?

The most common cause isn't the writing quality of the content, it's structure. A single page trying to cover every service you offer gives Google and AI systems nothing specific to point to. A dedicated page per real service, in your real city, with real details about how you actually treat that condition, gives both classic search and AI systems something concrete to match a patient's question against. The second most common cause is stale technical basics: missing schema markup, slow load times, or a site that hasn't been touched since it launched.

Can I get my Google reviews to show up instead of Yelp, or highlight certain reviews?

You can't force Google to hide a competing platform's reviews, but you can control what shows up on your own site and ads, and you can influence which reviews carry weight with AI systems. Reviews that mention a specific service, a specific outcome, or specific language a patient used are worth far more than a five-star rating with no text, both for a human deciding whether to book, and for an AI system trying to match your practice to a specific patient question.

How long does it take to start showing up in AI answers?

It depends on how far behind the fundamentals are. If your Google Business Profile is incomplete or your website has no dedicated service pages, there's real structural work before AI visibility is even possible. Once the fundamentals are in place, we've seen practices go from invisible to being named in AI answers within a few months, but it compounds instead of switching on overnight, and it keeps compounding as review volume and content depth grow.
